Servants Health Centre collects personal information so that we are able to provide health services.

 

The personal information we collect includes your:

• name

• contact information

• personal representative

• National Health Index number or NHI

We keep your information safe by storing it securely in an approved electronic patient record. Only authorised staff are permitted to have access to this for the purposes of providing you with health services. This information is kept in accordance with the principles of the Health Information Privacy Code (HIPC) 2020, which sets out the rules that govern how health information about identifiable individuals is collected, used, stored, and shared by health agencies in New Zealand.

 

By following the requirements of the HIPC this makes sure that your personal health information is handled with care and confidentiality, protecting your privacy while allowing necessary information sharing for healthcare purposes.

 

You have the right to ask for a copy of any personal information we hold about you, and to ask for it to be corrected if you think it is wrong. If you would like to ask for a copy of your information, or to have it corrected, please contact Servants Health Centre, 100 Princes Street Dunedin, or by calling us on (03) 477 7040.

What is HealthOne?

 

HealthOne is a South Island based secure electronic record that allows registered healthcare providers, directly involved in your healthcare, to quickly access information such as your test results, allergies, medications, GP summaries and hospital information.
